Intake Process

1. Submit intake form to ardenhouseidaho@gmail.com
2. If IDHW caseworker referring, please utilize Arden House email provided by congregate care unit (J******@ardenhouseidaho.com)
3. If outside entity/parent, please call 208-358-9034 to schedule an assessment. 

*Please note that authorizations submitted to Medicaid (Magellan) could take up to two weeks to process.
